Client Agreement

All Session fees are paid in advance. Choose a single session, multiple sessions (discounted) or a monthly contract (if applicable). Coaching sessions require a minimum of 24-hour notice to cancel or reschedule a call. If you do not call within 10 minutes of your scheduled session time the session will be forfeited, and you will be charged for the missed session. Once a session (or a series of sessions) has been purchased, it is not refundable nor is it transferable for any reason. Coaching sessions will never expire. 

New Clients:

Please submit a New Client Intake Form online 24-hours in advance of your session. If you do not submit your new client form at least 48-hours prior to your session, the session will be forfeited. By purchasing a session and agreeing to the terms herein you acknowledge that you have never accepted domestic violence services from Kristina Lindsay at any domestic violence agency.

Existing Clients:

Complete your online Session Prep Form at least 48-hours prior to your session. If you do not submit your new client form at least 48-hours prior to your session, the session will be forfeited, and refunds will not be given, as the time was reserved for you.  

Termination of Services:

Coaching services can be terminated by either party at any time and for any reason. Upon receipt of an email notification canceling services, the coaching relationship will be discontinued effective immediately. Confidentiality:

Kristina Lindsay (Coach) is a mandated reporter and a Certified Domestic Violence Professional (CDVP). Session content is confidential unless abuse of child, a severely disabled adult or elderly individual is disclosed. Additionally, if you (Client) disclose suicidal ideation or a plan to hurt someone else; confidentiality will not apply. Our communications may be privileged due to Coach’s status as a CDVP: consult with your own attorney regarding this. It may be that communications are discoverable during a divorce, custody, or other legal proceeding. To be clear, “Coach” is neither an attorney nor therapist and must respond to requests for communications, documents, or provide testimony to the extent compelled to by applicable law. “Confidential Information” means any information reasonably understood to be confidential, whether written or oral, tangible or intangible, disclosed by one party (“Disclosing Party'') to the other party. (“Receiving Party”), or which may otherwise become known to the Receiving Party regarding the Disclosing Party. During and after the Term, the Receiving Party shall hold in trust and confidence, and take reasonable precautions to protect all Confidential Information. Notwithstanding the foregoing, Confidential Information shall not include information that: (a) is generally available to the public other than through disclosure by the Receiving Party; or (b) was known to the Receiving Party prior to its disclosure by the Disclosing Party, or becomes available to the Receiving Party after such disclosure, in either case from a third party source that lawfully obtained the information. The Receiving Party may disclose Confidential Information to the extent compelled by judicial or governmental process; provided the Receiving Party gives the Disclosing Party prompt notice thereof so that the Disclosing Party has the opportunity to obtain a protective order or otherwise oppose the disclosure.

How to Get the Most from your Coaching Experience:

  • Indicate on the Session Prep Form what you want to take away from each session. 

  • Show up to each session on time and ready to engage.

  • Be prepared to complete any tasks given to you and integrate what you learned during the session.

  • Be open-minded and willing to change any beliefs and/or behaviors that are unproductive and will hinder your success.

  • Prepare any emails/texts or additional documentation before the session to save time. Any prep time your coach has to incur outside of the session call with be deducted from client's session time. 

  • If you call in for your session and get voice mail, your coach will call you back shortly and it will not count against your session time.

  • Make sure to Cancel or Reschedule a minimum of 24-hours in advance of session time to avoid forfeit of your session fee. 

  • Make sure to call or log into Microsoft Teams no later than 10 minutes after the start of your scheduled session time to avoid forfeit of your session fee.

Client Agreement:

By signing below you acknowledge that Kristina Lindsay and Empowered Outcomes Consulting, LLC are not qualified to provided mental health or legal services. Kristina Lindsay and Empowered Outcomes Consulting LLC are able to provide supportive strategy during high conflict divorces or custody cases, but do replace a therapist or an attorney. I also acknowledge that I have thoroughly read through each section of the client information and policies form and agree to its terms.
